Registration fees include all meals and hotel accommodations. Travel expenses not included.
Deposit Payment – Please note your deposit payment is non-refundable. Balance payments are due no later than February 17, 2022. If balance payment not received by this deadline, you will forfeit your registration and deposit.
Cancellation Policy – Written cancellation immediately after registration: $500.00 US cancellation fee per person. Cancellation after March 3, 2022: 50% of total workshop cost per person. No refunds after March 28, 2022. No refund for unused portions of the tour, including but not limited to, missed meals, hotel nights and sightseeing.
COVID-19 Vaccination – All workshop attendees must provide proof that they are fully vaccinated and have received a booster shot if they are eligible for one prior to their arrival at the workshop venue.
Waiver of Liability – By attending this workshop, you agree to sign a release to forever discharge CMED, LLC from all claims of any kind for any injury or illness that may arise as a result of your attendance. If you decide not sign this release, you are effectively cancelling your reservation and will be liable for the cancellation fees spelled out above. This release will be provided to you as part of the final registration process 6 weeks prior to the beginning of the workshop.
